GRAFTON, Mass. - Kindergarten registration and initial screening is planned for early March in the Grafton Public Schools.

Kindergarten Registration and Phase I of the screening process (hearing and speech/language screenings) will take place on March 7-8. Children who are five years old on or before September 1, 2012 are eligible to enter kindergarten.

Parents should register children at their home school. The registration process will take place as follows:

Wednesday March 7, 2012 - Children whose last name begins with the letters A-L
        9:15-12:00  A-F
        1:00-3:00 G-L
Thursday March 8, 2012 - Children whose last name begins with the letters M-Z
        9:15-12:00 M-R
        1:00-3:00 S-Z

The Commonwealth of Massachusetts Attendance Regulations requires that all children who
have attained the age of six years must be registered and entered in a school program. Parents do have the option of
not entering their children into kindergarten until they have reached the age of six years.

The child must be present for registration and the following must be provided:

  • An original birth certificate (no photocopy)
  • Proof of residence, occupancy, and photo ID. (see attached Registration Document Chart)
  • Medical Information: Physical history and physical exam within the last 12 months.
  • Evidence of blood lead level screening.
  • Proof of immunization against diphtheria, tetanus, whooping cough, measles, mumps, rubella, polio (unless exempt for medical or religious reasons).
  • Proof of Hepatitis B series 1, 2, and 3 immunization.
  • Proof of Varicella vaccination having been administered or physician documentation that your child has had chicken pox.
  • Proof of Hib series vaccination.
  • Proof of vision and steropsis testing.
